IQM Business Investigations Client's Work Order

You may request any of our investigative services using our convenient online request forms. The forms are broken down into the following areas:

"Full Five Years - Nuclear Cases" - Are all inclusive.

"Applicant's Authorization and Release" - This is your first step to initiating an investigation of a possible candidate. This form must be printed out and signed by the candidate and kept on file at your company site. After this is completed you may submit this form to us via the Internet using your PIN and supplying the candidate's name and the date in the required fields.

"Candidate's Basic Personnel Security Questionnaire" - This form also has the required  Authorization and Release form.  Therefore if you are using this form it is not not necessary to print out the "Applicant's Authorization and Release form described above.  However, it may be easier to print it out.  In either case, the release must be signed and on file. The rest of the data should then be input and this form submitted. This will give us necessary information to perform our background checks.

Once you have completed the previous steps you to expand the investigative service. You may break the services down within the following areas.  This is described as a Special Elements Case.  In the case of the full five year nuclear investigation, use the complete work order. 

"Candidate's Criminal and Driving History" - Use this form to check for a candidate's criminal record and/or driving history.

"Candidate's Employment Activity Detailed" - Use this form to verify a candidate's previous employment record and inquire about their attitudes and preferences concerning drug and alcohol abuse.

"Candidate's Educational, Training and Military Attainment" - Used for background checks into candidate'SE educational and/or military background as well as any accredited training they may have received.

"Verification of the Candidate's Character and Reputation in the Community" - Verify through references the candidate's character and reputation in the community.

"Test Score Transmittal Sheet" - Used in conjunction with the "Personal Attitudes Inventory" test we provide to you.

"Polygraph and Drug Abuse Screens - Work Order" - This work order contains specialized investigative services, like polygraph or drug abuse screens, not requiring a separate form.

"Special Element - Nuclear Security Access Authorization Background Investigation Work Order" - This work order contains our complete investigative services and leads to all necessary forms for an update. This is called a "Special Elements" case.

"Full Five Years - Nuclear Security Access Authorization Background Investigation Work Order" - Select this work order for an all inclusive form to initiate a full five years background check to gain unescorted access into a nuclear power plant. The application must be authorized by the security personnel controlling the access. The investigation may be divided into two segments: (a) Temporary Report; (b) Final Report.
