IQM's Personnel Inquiry Processing Kit

Registered clients will be electronically issued a pass word for access to our proprietary systems. The Personnel Inquiry Processing Kit can be ordered.  It consists of sample policies and procedures, forms, written tests and drug and alcohol abuse testing kits for either urine or hair specimens. Most of the forms can be completed on line and securely transmitted to us electronically for a major discount. The client may choose to use the traditional paper forms and fax submission.  The client has a choice for "Investigation Report" delivery. They may be submitted to the client's designated location by express mail, fax for a fee or attached to an E-Mail at no charge.

The Electronic Relationship

(A) "Client's Work Order With Candidate's Release and Basic Data." On each applicant's Work Order, the client is requested to specify the investigative actions desired for that case. Standard package investigations may be agreed to between us and have a fixed price. Alternatively, special elements may be selected. Stock Investigative Elements are identified below and may be entered into the Work Order by topic with an explanation. For example, type into the dialog box: "Please do a criminal history search in Denver, Colorado and Dallas, Texas."  Fees are charged by the Element and invoiced on a net 15 day basis for Registered clients.  Specifications for the Elements and Packages are set out in the Registration agreement.  Basic data, to positively identify the client's candidate, is also asked for on the Work Order. Often that will be sufficient information to conduct the requested investigative elements such as credit checks or driving history and criminal court checks, if the candidate claims that no record exists. Education, military, employment, or criminal history details require using the advanced forms.

(B)"Authorization and Release."   This paragraph also appears on the Work Order above the personal true identity information. The paragraph, which has been legally reviewed, confirms that the applicant understands this condition of employment. The Work Order is a consolidated form, where as this link will lead you to two stand alone Authorization and Release Forms, which are intended to be printed out and used for a transmittal or file copies.  The second form is required by the Fair Credit Reporting Act as revised in 1997. The client, when using the forms, is both complying with the law and informing the candidate of the existence of a security plan thus putting them on notice that a background investigation is to be initiated. Some custodians of records require a signed Authorization and Release before providing information.  Therefore, IQM may need to have a signed copy faxed to us. We will ask for it if we do.

(C) "Candidate's Personnel Security Questionnaire." There are several levels of Questionnaires. The Work Order serves as the first section of each of the Personnel Security Questionnaire(s) with one exception. There is a consolidated form for regulated investigations leading to a security clearance. The main body of these questionnaires list   specific and detailed questions about the candidates's education, jobs, residences, military service and names of people and institutes that can verify the statements. Essentially, the investigation seeks to verify the candidate's assertions through interviews and searches of records. Investigative questionnaires are used to standardize our agents' inquiries. Investigators' records of action are noted in a Serialized Case Books which are retained for audit.

(D) "Personnel Selection Inventory Test." This pencil and paper test measures the behavioral traits of the applicant in compliance with the Americans With Disabilities Act (ADA). It is very useful in determining if the applicant's likes and dislikes are compatible with your organization. A supply of serialized Personnel Selection Inventory test booklets may be ordered by registered clients via E-Mail. They will be delivered overnight by express mail. The applicant's test answers form a numerical pattern. The client records the numerical pattern on a form. That form may be downloaded from IQM to serve as a draft. That form, when completed, may be entered on line or faxed to IQM who will quickly score the tests and return the results as an E-mail attachment for use in the interview process.

(E) "Drug Abuse Screening Kit." A Drug Free Workplace system, which may be ordered via E-Mail, has as its keystone both a urine and hair collection, documentation, and transmittal kit for express delivery of the specimen to the lab. This program satisfies the NIDA, which sets the federal regulatory standards. The most efficient collection technique is for the collection to be done at the client's site. The hair specimen is ideal for this. A modest amount of training will be required but it can be done remotely. When internal collections are not convenient, and we can travel to a site for multiple collections or arrange for a local clinic to collect and dispatch specimens. IQM serves as Employee Assistance Advisor.
